Chore Services
Chore services are essential for maintaining a safe and functional living environment, particularly for individuals with limited mobility or other challenges. Our Chore services encompass a range of heavy household activities, including washing floors, windows, and walls; securing loose rugs and tiles; rearranging furniture for accessibility; and addressing outdoor maintenance such as snow and leaf removal and yard upkeep. With attention to detail and a focus on safety, our Chore services ensure that the home remains a secure and welcoming haven for our clients.
Respite
Respite services are provided to supervise and support individuals living in private homes on a short-term basis for planned or emergency situations, giving the person(s) normally providing care a period of relief that may be scheduled or due to an emergency.
Transportation
This service enables individuals to access services and activities specified in their approved service plan. We understand the importance of accessibility and independence, and our mission is to provide reliable and inclusive transportation solutions for our clients.
Companion
Companion services are direct services provided to individuals age 18 and older who live in private home for the purpose of providing supervision or assistance that is designed to ensure the individual’s health, safety, and welfare or to perform activities of daily living for the individual.
HOME & COMMUNITY HABILITATION
The service assists individuals in acquiring, maintaining, and improving the skills necessary to live in the community, to live more independently, and to participate meaningfully in community life. This service consists of assistance, support, and guidance (physical assistance, instruction, prompting, modeling, and reinforcement) in the general areas of self-care, health maintenance, decision-making, home management, managing personal resources, communication, mobility and transportation, relationship development and socialization, personal adjustment, participating in community functions and activities and use of community resources.
Community Participation Support
Community Participation Support provides opportunities and support for community inclusion for building interest in and developing skills and potential for competitive integrated employment. Each individual is offered opportunities and needed support to participate in community activities that are consistent with the individual’s preferences, choices, and interests. This service supports individuals in developing and sustaining a range of valued social roles and relationships, building resources and experiences in the family community, increasing independence, increasing potential for employment, and experiencing meaningful community participation and inclusion.
